However, for individuals with type 1 diabetes, this type of exercise carries hidden risks that are often overlooked. Understanding these risks is essential to safely enjoy the benefits of high-intensity workouts without compromising blood sugar control or overall health.




Why High-Intensity Training Appeals to People with Type 1 Diabetes


People with type 1 diabetes often face challenges managing blood sugar levels during physical activity. High-intensity training can be appealing because it:


  • Requires less time than traditional workouts

  • Can improve insulin sensitivity

  • Supports cardiovascular and muscular health

  • Offers variety and motivation in exercise routines


Despite these benefits, the intensity and metabolic demands of HIT can cause unpredictable blood sugar fluctuations, which may lead to dangerous situations if not managed carefully.


Blood Sugar Challenges During High-Intensity Training


Unlike moderate exercise, which generally lowers blood sugar steadily, high-intensity training can cause rapid and sometimes opposite effects on glucose levels. The body’s response to intense exercise includes:


  • Release of stress hormones like adrenaline and cortisol

  • Increased glucose production by the liver

  • Temporary insulin resistance in muscle cells


These responses can cause blood sugar to spike during or immediately after exercise, followed by a delayed drop hours later. This rollercoaster effect makes it difficult to predict and manage glucose levels.

A person with type 1 diabetes starts a 20-minute HIIT session. During the workout, adrenaline causes their blood sugar to rise from 120 mg/dL to 180 mg/dL. After finishing, insulin sensitivity improves, and blood sugar may drop to dangerous lows overnight if insulin or carbohydrate intake is not adjusted properly.


Risk of Hypoglycemia and Hyperglycemia


Two main risks arise from high-intensity training for people with type 1 diabetes:


  • Hypoglycemia (low blood sugar): Can occur during or after exercise, especially if insulin doses are not adjusted or carbohydrate intake is insufficient. Symptoms include dizziness, confusion, sweating, and in severe cases, loss of consciousness.

  • Hyperglycemia (high blood sugar): Can happen during intense exercise due to stress hormone release. High blood sugar levels can cause dehydration, fatigue, and long-term complications if frequent.


Both conditions require careful monitoring and management to avoid emergencies.


Strategies to Reduce Risks


Managing the hidden risks of high-intensity training involves preparation, monitoring, and adjustments. Here are practical tips:


  • Check blood sugar before, during, and after exercise: Use continuous glucose monitors (CGMs) or frequent finger sticks to track changes.

  • Adjust insulin doses: Work with a healthcare provider to modify basal and bolus insulin around workout times.

  • Plan carbohydrate intake: Consume fast-acting carbs if blood sugar is low before starting, and have snacks ready for after exercise.

  • Stay hydrated: Dehydration can worsen blood sugar control and increase risk of complications.

  • Start gradually: Build up intensity and duration slowly to understand how your body responds.

  • Avoid exercising alone: Have someone nearby who knows how to help in case of hypoglycemia.


When to Avoid High-Intensity Training


Certain situations increase the risks of HIT for people with type 1 diabetes:


  • If blood sugar is above 250 mg/dL with ketones present, exercise should be postponed.

  • During illness or infection, when blood sugar control is unstable.

  • If you experience frequent hypoglycemia unawareness or severe lows.

  • When new to exercise, without medical guidance or proper monitoring tools.


Consulting with an endocrinologist or diabetes educator before starting a high-intensity program is crucial.

High-intensity training can be part of a healthy lifestyle for individuals with type 1 diabetes, but it requires respect for the body's signals and careful planning. The key is to balance the benefits of improved fitness with the need for stable blood sugar levels.
